The initial part of First Chapter begins with prayers and introduces the Divine Lineage of the wisdom of Ayurveda.
This describes how ancient sages noticed the increasing tendency of attracting disorders in common people and then held a symposium in Himalayan region and decided to bring Ayurveda wisdom for the masses.

” Athāta upakalpanīyamadhyāyaṃ vyākhyāsyāmaḥ || 1||”

” Iti ha smāha bhagavānātreyaḥ || 2||”

” Iha khalu rājānaṃ rājamātramanyaṃ vā vipuladravyaṃ vamanaṃ virecanaṃ vā pāyayitukāmena bhiṣajā prāgevauṣadhapānāt sambhārā upakalpanīyā bhavanti, samyak caiva hi gacchatyauṣadhe pratibhogārthāḥ, dhyāpanne cauṣadhe vyāpadaḥ parisaṃkhyāya pratīkārārthāḥ; na hi sankṛṣṭe kāle prādurbhūtāyāmāpādi satyapi kryākrye sukaramāśu sambharaṇamauṣadhānāṃ yathavaditi || 3||”

” Evaṃvādinaṃ bhagavantamātreyamgniveśa uvāca-nanu, bagavan ! ādāveva jῆānavatā tathā pratividhātavyaṃ yathā prativihite sidhyedevauṣadhamekāntena, samyak pryonimittā hi sarvakarmaṇāṃ siddhirīṣṭā, vyāpaccāsamyakprayoganimittā; atha samyagayamyak ca smārabdhaṃ karma sidhyati vyāpaghate vāniyamena, tulyaṃ bhavati jῆānamajῆāneneti || 4 ||”

” Tamuvāca bhagavānatreyaḥ śakyaṃ tathā pratividhātumasmābhirasmadvidhairvāvyagniveśa ! yathā prativihite sidhyedevauṣadhamekānten, tacca prayogasauṣṭhavamupadeṣṭuṃ yathāvat; na hi kaścadasti ya etadevamupadiṣṭamupadhārayitumutsaheta, upadhārya vā tathā pratipattuṃ prayoktuṃ vā; sūkṣamāṇi hi doṣabheṣajadeśakālabalaśarīrāhārasātmyasatvaprakṛtivayasāmava- sthāntarāṇi, yānyanucintyamānāni vimalivipulabuddherapi buddimākulīkuryuḥ kiṃ pranaralpabuddheḥ, tasmādubhayametadhathāvadupadekṣyāmaḥ-samyakprayogaṃ cauṣadhānāṃ, vyāpannānāṃ ca vyāpatsādhanāni siddhiṣūttarakālkam || 5||”

” Idāṇīṃ tāvatsambhārān vividhānapi samāsenopadekṣyāmaḥ; tadhathā –dṛḍhaṃ nivātaṃ pravātaikadeśaṃ sukhapravicāramanupatyakaṃ dhūmātapajalarajasāmanabhigamanīyamaniṣṭānāṃ ca ṣabdasparśarasarūpagandhānāṃ sodapānodūkhalamusasalavarcaḥsthānasnānabhūmimahānasaṃ vāstuvidhākuśalaḥ praśastaṃ gṛhameva tāvat pūrvamupakalpayet || 6||”

” Tataḥ śīlaśaucācārānurāgadākṣyaprādakṣiṇyopapannānupacāra-kuśalān sarvakarmasu paryavadātāna sūpaudanapācakasnāpakasaṃvāhakotthāpakaṃveśakauṣadhapekāṃśca paricārakān sarvakarmasvapratikūlān, tathā gītavāditrollāpakaślokagāthākhyāyikektihāsapurāṇakuśalān- bhiprāyajῆānanumatāṃśca deśakālavidaḥ pāriṣadhāṃśca, tāthā lāvakapiῆjalaśaśahariṇaiṇakālapucchakamṛgamātṛkorabhrān, gāṃ dogdhrīṃ śīlavatīmanāturāṃ jīvadvatsāṃ suprativihitatṛṇaśaraṇapā- nīyāṃ, pātryācamanīyodakoṣṭhamaṇikaghaṭapiṭharaparyogakumbhīkumbha- kuṇḍāśarāvadarvīkatodaῆcanaparipacanamanthānacarmacelasūtrakārpā- sorṇādīni ca, śayanāsanādīni chopanyastabhṛaṃgāpratigrahāṇi suprayuktāstaraṇottarapracchadopadhānāni sopāśrayāīṇi saṃveśanopaveśanasnehasvedābhyaṃgapradehapariṣekānulepanavamanavire- canāsthāpanānuvāsanaśirovirecanamūtroccārakamaṇāmupacāra- sukhāni, suprakṣālitopadhānāśca suślakṣaṇakharamadhyamā dṛṣadaḥ, ṣastrāṇi chopakaraṇārthāni, dūmanetraṃ ca, bastinetraṃ cottarabastikaṃ ca kuśahastakaṃ ca, tulāṃ ca, mānabhāṇaḍaṃ ca, Ghṛatatailavasāmajjakṣaudraphāṇitalavaṇendhanodakamadhuśīdhusurāsau- vīrakatuśodakamaireyamedakadadhimaṇḍodaśvaddhānyāmlamūtrāṇi ca, tathā śāliṣaṣṭikamudgamāṣayavatilakulatthabadaramṛdvīkākāśmaryapa- rūṣakābhayāmalakabibhītakāni, nānāvidhāni ca snehasvedopakaraṇāni dravyāṇi, tathaivordhvahrānulomikobhaya- bhāῆjisaḍgrahaṇīyadīyapācanīyopaśamanīyavātaharādisamā-khyātāni cauṣadhāni; yaccānyadapi kiṃcid vyāpadaḥ parisaṅkhyāya pratīkārārthamupakaraṇ vidhāt, yacca pratibhogārtha, tattadupakalpayet || 7|| “

” Tatastaṃ prarūṣaṃ yathoktābhyāṃ snehasvedābhyāṃ yathārhamupapādayet | taṃ cedasminnantare mānasaḥ śārīro vā vyādhiḥ kaścittvīvrataraḥ sahasābhyāgacchettameva tāvadasyopāvartayituḥ yatet | tatastamupāvartya tāvantamevainaṃ kālaṃ tathāvidhenaiva karmaṇopācāret || 8||”

” Tatastaṃ purūṣaṃ snehasvedopapannamanupahatamanasamabhisamīkṣya sukhoṣitaṃ suprajīrṇabhaktaṃ siraḥsnātamanuliptagātrāṃ sragviṇamanupahatavastrasaṃvītaṃ devatāgnidvijagurūvṛddhavaidhānarcitavataṃ iṣṭai nakṣatratithikaraṇamuhūrte kārayitvābrāhamṇān svastivācanaṃ prayuktābhirāśīrabhimantritāṃ madhumadhukasaindhavaphāṇitopahitāṃ madanaphalakaṣāyamātrāṃ pāyayet || 9||”

” Madanaphalakaṣāyamātrāpramāṇaṃ tu khalu sarvasaṃśodhamatrāpramāṇāni ca pratipuruṣamapekṣitavyāni bhavanti; yāvaddhi yasya saṃśodhanaṃ pītaṃ vaikārikadośaharaṇāyopapadhate na cātiyogāyogāya, tāvadasya mātrāpramāṇaṃ veditavyaṃ bhavati
|| 10||”

” Pītavantaṃ tu khalvenaṃ muhurtamanukāṅkṣet | tasya yadā jānīyāt svedaprādurbhaveṇa doṣaṃ pravilayanamāpadhamānaṃ, lomaharṣeṇa ca sthānebhyaḥ pracalitaḥ, kukṣisamādhmāpanena ca kukṣimanugataṃ hrallāsāsyastravaṇābhyāmapi cotordhvamukhībhūtam, athāsmai jānusamamasambāndhaṃ suprayuktāstaraṇottarapracchadopadhānaṃ sopāśrayamāsanamupaveṣṭuṃ prayacchet | pratigrahāṃścopacārayet-lālāṭapratigrahe pārśvopagrahaṇe nābhiprapīḍane pṛṣṭhonmadane cānapatrapaṇīyā suhradonumatāḥ prayateran || 11||”

” Athainamanuśīṣyāt- vivṛtauṣṭhatālukaṇṭho nātimahatā vyāyāmena vegānudīrṇānudīrayana kiῆcidavanamya grīvāmūrdhvaśarīramupavegamapravṛttān pravartayan suparilikhitanakhābhyāmaṃgagulibhyāmutpalakumudasaugandhikanā-lairvā kaṇṭhamabhisprśan sukhaṃ pravartayasveti sa thāvidhaṃ kuryāt | tatosya vegān pratigrhagatānavekṣetāvahitaḥ; vegaviśeṣadarśanāddhi kuśalo yogāyogātiyogaviśeṣānupalabhet, vigaviśeṣadarśī punaḥ kṛtyaṃ yathārhamavabudhyeta lakṣaṇen; tasmādvegānavekṣetāvahitaḥ || 12||”

” Tatrāmūnyayogayogātiyogaviśeṣajῆānāni bhavanti tadhathā-apravrttiḥ kutaścat, kevalasya vāpyauṣadhasya vibhraṃśo vibandho vegānāmayogalakṣaṇāni bhavanti; kāle pṛavrttiranatimahatī vyathā yathākramaṃ doṣharaṇaṃ svayaṃ cāvasthānamiti yogalakṣaṇāni bhavanti, yogena tu doṣapramāṇaviśeṣeṇa tīkṣṇamrdumadhyavibhāgo jῆeyaḥ; yogādhikyen tu phenilaraktacandrikopagamanamityatiyogalakṣaṇāni bhavanti | tatrātiyogāyoganimittānimānupadravān vidhāt-ādhmānaṃ parikarttikā parisrāvo hradayopasaraṇamaṃgagraho jīvādānaṃ vibhraśaḥ stambhaḥ klamaścetyupadravāḥ || 13||”

” Yogena tu khalvenaṃ charditavantamabhisamīkṣya suprakṣālitapāṇipādāsyaṃ muhurtamāśvāsya. Snaihikavairecanikopaśamanīyānāṃ dhūmānāmanyatamaṃ sāmarthyataḥ pāyayitvā, punarevodakamupaspaśayet || 14||”

” Upaspṛṣṭodakaṃ cainaṃ nivātamāgāramanupraveśya saṃveśya cānuśiṣyāt – uccairbhāṣyamatyāsanamatisthānamaticaṅ kramaṇaṃ krodhaśokahimātapāvaśyāyātipravātān yānayānaṃ grāmyadharmamasvapanaṃ niśi divā svapnaṃ virūddhājīrṇāsātmyākālapramitātihīnaguṛūviṣamabhojanavegasaṃ-dhāraṇodīraṇamiti bhāvānetān manasāpyasevamānaḥ sarvamaho gamyasveti | sa tathā kuryāt || 15||”

” āthainaṃ sāyāhre pare vāhri sukhodakapariṣiktaṃ purāṇanāṃ lohitaṣālitaṇḍulānāṃ svavaklinnāṃ maṇḍspūrvāṃ sukhoṣaṇāṃ yavāgūṃ pāyayedagnibalamabhisamīkṣya, evaṃ dvitīye tṛtīye cānnakāle; caturthe tvannakāle tathāvidhānāmeva śālitaṇḍulānāmutsvinnāṃ vilepīmuṣṇodakadvitīyāmasnehalavaṇāmalpasnehalavaṇāṃ vā bhojayet eva paῆcame ṣaṣṭhe cānnakāle; saptame tvanṇnakāle tathāvidhānāmeva śālināṃ dviprasṛtaṃ susvinnamodanamuṣṇodakā- nupānaṃ tanunā tanusnehalavaṇopapannena mṛdgayūṣeṇa bhojayet, evamaṣṭame navame cānnakāle; daśame tavannakāle lāvakapiῆjalādīnamanyatamasya maṃsarasenaudakalāvaṇiken nātisāravatā bhojayeduṣṇodakānupānam, evamekadāśe dvādaśe cānnakāle; ata ūrdhvamannaguṇān krmeṇopabhuῆjānaḥ saptarātreṇa prakṛtibhojanamāgacchet || 16||”

” Athainaṃ punareva snehasvedābhyāmuppādhānupahatamanasamabhisamīkṣya sukhoṣitaṃ suprajīrṇabhaktaṃ kṛtahomabalimaṃgalajapaprāyaścittamiṣṭe tathinakṣatrakarānāmuhūrte brāhamṇān svasti vācayitvā trvṛtkalamakṣamātraṃ yathārhāloḍanaprativinītaṃ pāyayet prasamīkṣya doṣabheṣajadeśakālabalaśarī-Rāhārāsātmyasattvaprakṛtivayasāmavasthāntarāṇi vikārāṃśca | samyagviriktaṃ caunaṃ vamanoktena dhūmavarjena vidhinopapādayedābalavarṇapratilābhāt | balavarṇopapannaṃ caunamanupahatamanasamabhisamīkṣya sukhoṣitaṃ suprajīrṇabhaktaṃ śiraḥsnātamanuliptagātraṃ sragviṇamanupahatavastrasaṃvītamanarū- pālaṃkārālaṅkṛtaṃ suhrdāṃ darśyitvā jῆātīnāṃ darśayet, athainaṃ kāmeṣvavasṛjet
|| 17||”

” Anena vidhinā rājā rājamātrothavā punaḥ | yasya vā vipulaṃ dravyaṃ sa saṃśodhanamarhati || 18||”

” Daridrastvāpadaṃ prāpya prāptakālaṃ viśodhanam | pibet kāmamasaṃbhṛtya sambhārānapi durlabhān || 19||”

” Na hi srvamanuśyāṇāṃ santi sarve paricchadāḥ | na ca rogā na bādante daridranapi dārūṇāḥ || 20||”

” Yadhcchakyaṃ manuśyeṇa kartumauṣadhamāpadi | pattat sevyaṃ yathāśakti vasanānyaśanāni ca || 21||”

” Malāpahaṃ rogaharaṃ balavarṇaprasādanam | pītvā saṃśodhanaṃ samyagāyuṣā yujyate ciram || 22||”

” īśvarāṇāṃ vasumatāṃ vamanaṃ savirecanam | sambhārā ye yadartha ca smānīya prayojayet || 23||”

” Yatha praojyā mātrā yā yadayogasya lakṣaṇam | yogātiyogayoryacca doṣā ye cāpyupadravāḥ || 24||”

” Yadasevyaṃ viśuddhena yaśca saṃsarjanakrmaḥ | tat sarva kalpanādhyāye vyājahāra punarvasuḥ || 25||”

” Ityagniveśakṛte tantre carakapratisaṃskṛte sūtrasthāne kalpanācatuṣke upkalpanīyo nāma paῆcadaśodhāyaḥ || 15||”
